The disputed claims covered 39 CPT codes for high-complexity reconstructive procedures performed out-of-network, including CPT 19318 (Reduction Mammaplasty), CPT 15830 (Infraumbilical Panniculectomy), and CPT 15847 (Complex Abdominoplasty).

CollectionPro's submission focused on clinical evidence rather than billing data alone.
